Fax or email the referral form with supporting documentation. We contact the patient, schedule the visit and confirm it took place.
How to refer
- Download the referral form (PDF).
- Complete the patient information, referral type, specialty and priority.
- Fax it to 832-460-3110 with supporting documentation, or email it to intake@acephysicians.com.
- Our coordinator contacts the patient and schedules the visit.
Please include
- Patient demographics, a copy of the insurance card and driver's license
- The two most recent office notes
- Applicable diagnostic tests
- A recent history and physical

What can be referred
Specialty visits, surgical placement, diagnostic imaging (MRI, CT, X-ray, ultrasound, EMG) and durable medical equipment. We accept evaluate-and-treat, direct schedule, private pay, workers' compensation, Department of Labor and personal injury referrals.
